Current Developments in Endoscopic Suturing

Juliana Yang, David Lee, Ali Siddiqui

Endoscopic suturing is an emerging technique in the field of therapeutic endoscopy that is rapidly broadening its potential indications for use. In this review, we summarize the variety of indications as described under current literature and provide an overview of the latest developments in endoscopic suturing, with specific focus on the Overstitch™ endoscopic suturing system.

INFLAMMATORY BOWEL DISEASE: A PRACTICAL APPROACH, SERIES #98

Efficacy of Thiopurine Monotherapy in Ulcerative Colitis

Raina Shivashankar, Edward V. Loftus

The exact role of the thiopurines, azathioprine and 6-mercaptopurine in ulcerative colitis treatment algorithms has been debated. Specifically, their use in UC remains controversial since the evidence lies in small clinical trials. Here, we briefly review the evidence for thiopurine efficacy in UC.

GASTROINTESTINAL MOTILITY AND FUNCTIONAL BOWEL DISORDERS, SERIES #12

Simultaneous Robotic Placement of Gastric Electrical Neurostimulation System and Pyloroplasty in Gastroparesis Patients

Brian R. Davis, Alejandro Rios Tovar, Irene Sarosiek, Richard W. McCallum

In this report, we review surgical approaches to treat refractory gastroparesis focusing on simultaneous pyloroplasty and gastric electrical neurostimulator placement demonstrating the safety and efficacy of combining these procedures. We believe the combination of these techniques prevents the negative spiral of hospitalization and malnutrition that creates chronic debilitation and disability in gastroparesis.

From the Literature

Radiofrequency Ablation in Confirmed Low-Grade Dysplasia in Barrett's Esophagus

To determine the effectiveness of radiofrequency ablation (RFA) in clinical practice, this study compared the rate of progression of low-grade dysplasia (LGD) after RFA with endoscopic surveillance alone in routine clinical practice.

HBV Cirrhosis With Low Viral Load and HCC

To determine whether antiviral therapy (AVT) should be recommended for compensated cirrhosis patients with HBV infection and detectable, but low serum HBV-DNA levels, a retrospective cohort of 385 treatment-nai?ve, HBV-related compensated cirrhotic patients with a mean age of 51 years and with low HBV-DNA levels (less than 2000 IU/mL) was carried out for the development of HCC during a median of 5.6 years of follow-up.

NAFLD and Myocardial Dysfunction

NAFLD and heart failure are obesity-related conditions with high cardiovascular mortality and to study whether NAFLD is independently associated with subclinical myocardial remodeling or dysfunction among the general population, a cross-sectional analysis of 2713 participants from the multi-center, community- based coronary artery risk development in young adults (CARDIA Study), who underwent concurrent CT quantification of liver fat and comprehensive echocardiography with myocardial strain measured

Non-Celiac Wheat Sensitivity and Auto-Immune Markers or Disease

To evaluate the prevalence of auto-immune diseases (ADs), among patients with nonceliac wheat sensitivity (NCWS), investigation was carried out as to whether these patients carry antinuclear antibodies (ANA).

Dabigatran and Gastrointestinal Bleeding Protection with PPI and H2 Blockers

To determine whether gastroprotective agents (GPA) prevent gastrointestinal bleeding (GIB) in dabigatran users, a retrospective cohort study using a population- wide database managed by the Hong Kong Hospital Authority was carried out.

NAFLD In Adolescents Undergoing Bariatric Surgery

To investigate the prevalence of NAFLD in a multicenter cohort of adolescents undergoing bariatric surgery and the factors associated with it, a total of 242 adolescents undergoing bariatric surgery between March 2007 and February 2012 was carried out in five tertiary care centers in a multicenter prospective observational cohort study.

Ledipasvir and Sofosbuvir Plus Ribavirin for HCV with Advanced Liver Disease

A phase 2, open-label study was carried out, assessing treatment of the NS5A inhibitor ledipasvir, the nucleotide polymerase inhibitor sofosbuvir, and ribavirin in patients infected with HBV genotype 1 or 4.
